So randomly my server crashes and everyone loses connection to the server. Everything works...as in jobs/weapons, etc. Everything runs smoothly but it randomly crashes.
Here's the console output right before the crashes:

CreateFragmentsFromFile: 'user_custom/33/334662aa.dat' doesn't exist.
CreateFragmentsFromFile: 'user_custom/77/772c27b4.dat' doesn't exist.
CreateFragmentsFromFile: 'user_custom/a4/a45752ef.dat' doesn't exist.
CreateFragmentsFromFile: 'user_custom/8c/8c87000b.dat' doesn't exist.
CreateFragmentsFromFile: 'user_custom/15/154c5ec2.dat' doesn't exist.

Everything before it is normal stuff, like people joining, respawning, etc. Just out of the blue this stuff pops up and server goes under. I have no clue what the problem is, but my hoster believes it's "software issues" while I believe it could be corrupted data.
Any ideas? I have no clue what that file is.
Has your config file got "sv_kickerrornum 0"?

I don't have that in my server.cfg
EDIT: I put it in, and it still kicks everyone. Basically it's as if the entire server crashed and restarted.
Also, in terms of addons, we THINK this may be causing it, I have no clue though. They told us it was a persistent addon issue. I don't think playermodels would cause it.

Temporarily disable the addon and try.
Alright, we removed 3 addons, including the ones we thought were causing it but it was in vain. I don't know what addons to attempt to remove next as all of them I've seen working fine on other servers and there isn't a conflict between them that I know about. I personally think it's corrupt data or if we're missing some files?
Here's a list of the addons we use, any suggestions on what to temp remove next?
