Alright, so I have a GTX 765m 2 GB. Its not a bad card, I can get 40-50 fps on skyrim with ultra and 70 fps in COD Bo2 on max settings. However, GMod gets 40-70 fps. (15 fps with integrated graphics) I am confused because similar cards get 200+ fps. I do not overheat at all. Is gmod just incompatible with some cards? Thanks for any potential answers.

What video settings in Gmod are you using? As far as I know Vertical Sync kills FPS. If it's enabled, disable it. Try lowering video settings to medium/low and see how it runs.

I have vsync off, multicore rendering on (even though garry has disabled it for a while now) and the rest of the settings maxed out. I have tried lowering them to medium and low and it only increases fps by 10-15 so it is not worth it for the game to look so bad without much framerate improvement.
You GPU is definatly capable of running gmod, but I need your acctual CPU specifications.

I'm guessing that your CPU is the bottleneck of your system causing this lagg.
This means the CPU can not keep up with the GPU.
From your performance and if it is the CPU. I would take a guess that you have a i3 4th gen.
And from the speed its running at, I think that this is the bottleneck.

I have an i7. It goes from 2.4 to 3.4 ghz. (intel turbo boost)
Perhaps driver issue? Maybe you force some setting in NVidia control panel?
Have you tried updating?

VSync isn't "killing" the FPS, it's not a source of stutter in the Source engine. Having it on won't cause weird stuttering unless you're only just about running at 60FPS as it will keep skipping between 60/30.
As for your problem OP, are you absolutely positive it's running the nVidia GPU when in game and not the Intel one? Whilst the 4xxx Intel GPUs are pretty great, they aren't amazing and Source will think it can max itself on them. Source doesn't disagree with any modern GPUs, however driver issues might be a thing on that model still.
What do you have running in the background? How many addons are installed? I'm wondering if it's something in the game, seeing as your specs are fine.
Ram speeds are a bit low. Wouldn't cause problems though.

~1600MHz is about right for a gaming anything. You can get away with slower by quite a significant margin with DDR3.

Drivers are up to date, but the installation may be corrupt, so I may try reinstalling in the future. I have tried disabling addons, but I still get 50-60 fps in gm_construct. Definitley using the nvidia card (I get 15 fps with the intel GPU so I know the difference)

You are right. The reason I made two accounts is that I THOUGHT I clicked on "help and support" when I actually clicked on the discussion section. Since posting help there supposedly leads to a ban, I had to make another account and post here instead.
I fixed the problem. The solution is...
Quit out of GMod and play something else.
