I found (and this must be patch-related, not an addon because I removed all addons as a test) a bug where you enter bone editing mode, modify the spine of Breen (maybe any NPC, haven't tried), then he stretches up very far into the map. If you then kill Breen, it spits out the "hl2.exe has stopped responding" crash.
