When I saw this picture on the background of GMod, I was clueless on how to do this.

I know it isn't ropes. But seriously, how would I do this? Sorry if I sound stupid/dumb.

Not unless a person makes a good mod that makes you write stuff, it wont be possible without making a custom material or just photoshopping a picture.
It's not something real-time for sure.
Material edits/photoshop like the first reply said.
