Weird graphics issue and "Grab Brian"

System hardware specs:
-Intel Core 2 Duo CPU 2,26 & 2,27 GHz
-RAM: 4 GB
-Graphics card: ATI Mobility Radeon HD 4650
-Operating System: Windows Vista Home Premium 64 bit
-Laptop
(played on default graphics settings)

Addons installed:
-Adv. Duplicator
-Npc Scene

When I'm normally playing on my listen server, after some random time the whole graphics go crazy on me. To the server, that is. The console says: "***** WARNING: Transition table overflow. Grab Brian". And it keeps going crazy until I restart Gmod.

After changing and adding addons... nothing changed. It's happening to me since I ever installed Gmod, but only on my listen server. Help?

No, but, that Grab Brian, did you google it? I never heard about it.
