Pyramid Head (also called Red Pyramid Thing) is made up by James Sunderland (see pic: the guy on the left) to punish him for his sins... most bcs of the guilt and anger over murdering his own wife Mary. (see pic: Maria, chick on the left is Mary's more sexier other image)...
