A close up shot like this, you should have edited the blood in because the default/tf2 blood is ugly as sin.
All I have is Photoshot elements at the moment. Seems CS4 ran away...

What about the posing?
As far as I can tell, the posing looks decent. It's a close angle, so it's a little hard to judge.
I think you forgot to click "render" on the SDoF menu.

No, it's simple dof.
Don't use default blood or default muzzleflash.
And turn up Anti-Aliasing.
