I like the posing and composition. I think instead of vomit, or whatever that stuff is coming out of his mouth, you could add, say.. a bite, or a wound, and a few pock marks on his arms and face. Just a suggestion.

I've seen enough of this type of pose where one character wants the other one to shoot him due to infection to call it generic.
