In the read me all it says for installation is to copy gm_tmysql.dll to garrysmod/garrysmod/lua/includes/modules
I've done that but there are many other files in the download I have no Idea where to put.
Most of it's C++ Headers.
that IS all you have to do, the other stuff is in case you want to compile it yourself

you need to put libmysql in the same folder as srcds
