I could be wrong, but it appears to be you are only receiving grenade ammo from the grenade box. If you don't have the actual grenade weapon, you can't really do anything with the ammo. If you are using the default hl2 grenade, make sure the player has (or is given) 'weapon_frag' upon receiving the ammo.

I thought this was default in Half-Life 2? In sandbox opening this grenade box it gives you a grenade and 5 ammo. In my game-mode it gives you ammo but no grenade.
Updated first post, it appears to be with that snippet.
