Severe lage at 40+ players

Have you minimized addons? Yes, I have just playermodels, and tools, and a prop pack.
Are you on a garbage host? No. I'm on a dedicated box from NFO.

I am trying to get atleast 48, but i'd prefer to get as many as possible.

Any Ideas?
(I Installed DBugR, There's alot of "Net" and "Hooks" being called)

Around 40-48 players, the servers ping rises, mine from 60, to 200, everyones rises incredibly, and plop goes the server.

NFO is redonkulously un helpful with this.

Have you tried setting your tickrate to 33?
