Well from what I can see, you have no collisionjoints segment in the QC (this would make it a ragdoll)
As to the smoothing, it's a bug|feature in the max importer. The thing imports without smoothing groups but with vertex normals, which tend to break more often than not.
What you have to do is throw an edit normal modifier under the skin, reset them and then collapse it down. After that, you can set smoothing groups in the polygon segment.
(I recommend you use the autosmooth feature as that works better than just assigning 1 group to all polies and is far faster than manual assignment (though that will obviously give better results)
