What did I do wrong and what will fix it?

For whatever reason, the bones aren't attached completely. Head back into 3DS and make sure those leg/thigh bones are attached - I had the same problem with my Crimson Lance Mecha, and after re-attaching the bones, it fixed the problem.

So try linking the bones to each other?
Yeah; I'm assuming you're using 3DS Max to rig the models?

I tried it and now it just flails uncontrollably.
Now that I don't know the answer to, sorry. I think that might be a problem with the collision model, but don't quote me on that. I know jack-diddly about ragdolling, and unfortunately it seems that a lot of people on this forum either aren't interested in helping or don't know much either, I'm afraid =/
I've had all kinds of problems like this, sometimes it was because 2 bones in the ik chain had broken apart this meant me having to remake the whole skeleton again, hate to say it but ya may need to re do the skeleton, however i can help ya if ya want since i only ever do rigging for source.
