Error when testing an item in TF2. Need Assistance!

Hello,
I am working on an item for TF2 and have run into an error when I test it in game. The item is bound to his to his head and when it animates, the belt doesn't move, it stays in one place. That place is the location of where it is in the ref rig.
Here are some pictures to explain my situation.

It's not rigged to any bones.

I've rigged it to the "bip_spine_0" bone in 3Ds and it seems to work fine when I move in inside the program. Is there a different rig I should be using?

Oh, I bet you're using the old smd from the sdk. Not gonna work, you gotta decompile the Heavy mdl file and use the bones from that. Then they'll be named and placed correctly. Otherwise I'm totally wrong and have no idea what's wrong. There was a tutorial somewhere here on the forums for doing that.
do you know which .gcf that is located in?

Now I seem to be running into another problem decompling it.

Yeah, the decompiler is stupid. You have to create the folders it's trying to save stuff to. In this case, you'll need a /parts/ folder and inside that a /dmx/ folder wherever you're trying to extract the model. Also, there are some with lod files and those also get special separate folders as well. It's dumb, but after you do it a few times you won't even notice.
