It once banned a friend of mine because he was playing TF2 when an update shipped, meaning a couple of his .dlls were out of date, so VAC thought he was using teh-hax.

I've seen the same one multiple times: always payload, always brass beast heavy with speed hacks, crit hacks, no spread hacks, aimbot, wall hacks probably, and some hack that kept changing his name to other people's in the server so he couldn't get voted off.
