No. He doesn't die if he gets buffed all the way to 300 either.
You can't heal soldiers with the equalizer out.

I've done this multiple times.
If you have an ubered Soldier who does the taunt kill, they will have the explosion, fly upwards, and then fall. If they are not ubered and the Medic doesn't catch them on their fall, the fall damage will kill them.
You can actually have the rocket launcher out, switch to the equalizer, and then before the taunt animation begins switch back to the rocket launcher. That way you CAN be ubered while the Soldier does the taunt kill.

No higher than a real rocket jump.
Also, you have no control at all in the jump. It's straight up and maybe a few inches in the direction you choose. It's less fatal and generally a better idea to use regular rocket jumps.
