Seeing as you lose the game if breen is teleported before you blow the place to hell, he is dead. The spoilers are kinda unnecessary too.
The could do the same thing like in South Park when Chef's voice actor left so they replaced it with a synthesized voice if Breen did get to his host body.
Then again they probably won't do anything.

He was in a portal that exploded. Sure as hell seems survivable to me.

Well considering all that talk of being destroyed in more ways than is possible to be destroyed, I would say it's not.

Whether this character is dead or not will be revealed in episode 3.
It is possible he actually teleported, or at least a piece of him. It's also going to be revealed if he did end up with a new body.

They didn't replace his voice with a synthesized voice...they reused old sound bites of his voice.
I actually quite liked Breen, his voice actor & lines were brilliant, as well as the build up to the final confrontation with him.
It's just a shame that the battle itself was extremeley underwelming.
Although it would be funny if in Ep3 there was a rebel that looks like Breen but no one else in the game notices the similarity

If didn't die there, he's dead now.
That guy had life beyond voicing Breen.
Well first of the citadel exploded, got bathed in radiation, and exploded AGAIN. So I think he might be dead.

That would be in pretty poor taste considering Culp is dead and Chef's VA left because he was a scientologist.

"The last thing I saw was Breen falling" - Alyx. He is preety much dead.
Perhaps the aliens, namely the Vortigaunt, decisively teleported Dr. Breen and essentially YOU in the case of creating an anomaly in time which could potentionally destroy the Earth or City 17, which may answer (in theory) that he did survive the first explosion and the radiation but was teleported to avoid being killed by the second explosion. The reason or purpose for doing so might be revealed in Episode 3 of the Half-Life series.

Bringing Breen back would make the whole ending of HL2 seem pointless and dumb.
