I'm sure this has to do with Jack being not loyal, I did her mission and as she got into a fight with Miranda I took Miranda's side. Since then I have not gotten enough Paragon points to apologize to her.
Now, [b]do I buy the DLC to do paragon actions, no, I don't want to. But if I must to keep Samara alive then I will.[/b]
An alternative I thought of is, finding other missions to do, every galaxy I've got is 100%. Unless I have not bought some maps, in that case [b]can someone send me a link to the full list of galaxies? (I've googled, I'm sorry)[/b]
I've bought all the maps on the.. uh, the place where Liara is. Are there other maps to buy?

I think he chose Miranda and Samara with Miranda being the barrier. He chose Jack in the fight so Miranda hates him and died. Loyalty has a huge part in who dies/died.
I chose Samara and Grunt for this mission and had everyone in the Normandy loyal. In the end only The Doctor died because I never talked to him.

What, no Jack has no loyalty. But I played through again without Jack and Samara still died.
Jack has a loyalty mission.
She's loyal, then you can lose it in the fight with miranda. and you can't can't get it back without [I]x[/I] renegade.
The only ones that can to the barrier are jack or samara/morinth

Nope. Once your crew is taken you have a certain "time" you can save them. Missions or landing on a planet "advances time" and you show up too late. Remember when they say "if we showed up any later they would be dead"? Well they meant it.

Not that I remember. I do know there was a renegade option when you look through the sniper scope when you meet garrus. I never saw it because the scope was orange and the button is red.

Argh.. they should really make these actions larger and more noticable, maybe even a time stop, and you click if you want or don't want THEn it goes on. More than once I missed clicking paragon/renegade actions.

Its also largely chance and how you talked to them. (if at all)
I got it on my first try just from logical choices. I had all of them loyal before the Jack/Miranda fight as well.
