The government refuses to confirm if he is dead or alive. Accounts are conflicting, but it's very likely he's dead. This is the same guy that has done a series of videos advocating for elements of the Venezuelan Armed Forces to rebel against Maduro, he's also the one that famously hovered a helicopter above the TSJ's headquarters. Perhaps the most striking thing about this whole situation, is that Óscar Pérez made several posts to Instagram after getting made, including the moments before he declared he was surrendering and would not shoot, and after the government refused his surrender and seemingly wounded him using an RPG. The videos show him saying his last words, making a small speech to his family, shouting 'we are surrendering, do not shoot' all the way as he is getting fired upon. A Venezuelan friend of mine translated the videos and uploaded the transcriptions to his blog. His death is now confirmed, a picture of his corpse was leaked to Twitter, and it also confirmed his side of the story that he was getting hit by explosives. Won't be posting it here.
How many lives will be thrown against the government meat grinder? Venezuela doesn't deserve this, Maduro and his filthy hands have poisoned the well.
Maduro and his cronies deserve nothing short of a guillotine at this point. I honestly cannot voice how much I hate Maduro and his cronies. The amount of torture they are putting the Venezuelan people through should be something that the international community is willing to deal with in some way.

The moment where he got hit with an RPG and was bleeding from a side of his head, and the moment where he realised he wasn't actually making it out alive, I mean, they're hard to witness. After the failure of every single politician and diplomat in solving the Venezuelan crisis, people held hope that this guy had a shot at turning things around, even if it was a remote chance. Now that is over, all hope is lost. Things have changed for the worst, ultimately; the crisis rages on, only this time, we're sure it won't end without bloodshed. Maduro had people murdered in cold blood, streamed live, as they begged for mercy. More extreme attempts at deposing him will surely follow.
