I have all three STALKERS and have yet to venture far because i'm a huge vagine.
Most of the enemies in the "Tower of Latria" from Demon's Souls freaked me out (Though I suspected this was mostly due to the severe penalty of death)

The Dinosaurs from Dino Crisis. Whenever my Dad used to play it I always hid behind the sofa when he saw them.

God, the first parts of Ravenholm was some scary shit. Valve is good at making scary shit when it is a good time for it.
The whole nightmare level in Rayman 2 completely stopped me from finishing the game for YEARS. The slide ride with the master of nightmares (or whatever he's called in the English evrsion) was the ultimate NOPENOPENOPE moment for me. Similarly, if anyone remembers the Indiana Jones game about the tower of Babylon, those damn ice monsters crept me the fucked out as well at the time of playing. The lava level afterwards was even worse.
The screams of HL2's poison headcrabs were also pretty unnerving, but as it is with linear games I kinda got used to them after a couple of replays. But for a completely nonlethal enemy, the dev team did a pretty good job.
Metro 2033's librarian was only scary until I noticed it's actually killable and that running away from it is a pretty effective technique.
Amnesia monsters actually don't scare me at all, they're just ugly IMO.

Anything in Condemned: Criminal Origins. I used to be completely horrified of that game because of everything that popped out. I would always walk slowly, and shoot at random shit. I just beat it recently with only screaming a few times.
