What games are you trying to play music in?
Source Engine games had a built in MP3 player, but it was pretty poor.
In all seriousness, WMP or winamp are probably your best bet (especially if you have a reasonably decent multi-core CPU).
If you have a media keyboard, you can skip tracks using it.

Unless your talking about a program that plays sounds to other people on a server, like it would if you were talking down a mic. In Which case you should have been more descriptive.
Email Gabe Newell and ask him to add a media player to Steam that works with the overlay. I bet he won't do it, but considering he tries to respond to every email he gets, I'd like to see his response. Steam can already play media downloaded from the Steam site, so it would be a matter of simply adding the ability to add your own media to the media tab.
I think HLDJ would work for you.

I mean't locally, not over the mic, I want a program that allows me to change tracks, switch songs, or make playlists. not WMP or Winamp

Sorry, I didn't mean to sound like an ass. If you have a decent mouse with 4 or more buttons set to of the buttons to skip forward and skip track for WMP, that is what I do with my Microsoft Wireless Laser 5000.
All i've ever needed to play Music in games is WMP and my media keyboard, can control WMP in any full screen app.
A mouse is fine too.
I use either my mouse, or the Remote App with iTunes and my iPod.

You want a program that will allow you to see it while you are in-game and give you media control? Sadly I don't think that's possible, since the program would have to be installed into the game you are using in the first place.
I've been running winamp in the background for ages without any problems. Media keys allow me to change tracks/volume and I don't know why you would need to make playlists in game.
It would have to be something kinda like xfire but for music.
