I feel like re4 pulled off the perfect scary monster:

Regeneradores/Iron Maidens. It was the combination of a few things that make them perfect in my mind. The set up and tension that went into the first time you saw one lying dead and frozen on an operating table, and the realization that you had to heat up the room to progress but in so doing that thing on the table would probably thaw and attack you. Add onto that their ability to regenerate unless you shot them in specific areas, made them unwanted encounters for the rest of the game. And I remember one in particular, late in the game, coming out from behind a corner and scaring the crap out of me.
This fucker right here, always there when you're low on ammo and everything else.

Context: This happens in a mission early in the game, you basically get that the warp portals are safe areas(Really only had talismans of power, and save points). Then in a later mission, you come across one not expecting anything bad, and this scene happens.

vampire the masquerade bloodline, the monster at the porn filming spot and when you enter the ugly gore mansion. Theyre sound and all are just scary as fuck.
In the Galaxy News Radio Plaza. Just fought through what felt like a hundred super mutants.

Then a fucking bus explodes, two Paladins are killed instantly and...

HOLY SHIIIIT. Thank god I have a fighting chance with this mini nuke launcher..... and then I killed myself and all the other Paladins with it. Pretty scary and tense. I did not expect a huge ass boss monster to come out.
