to answer your question, third-party software exists for Source and GoldSource games, HLDJ being one example

You can manage this trough your audio settings, if your audio card supports it (many have it disabled for some reason).
onboard realtek soundcards have I think its called stereo mix, creative labs has what-u-hear, and theres a program called virtual audio cable that will do this

You can enable your stereo mix as a mic input through any options setting in any game, but I think to set it to a particular sound and not the whole output of your PC? I don't know.

Set it as mic.
I pretty much don't have stereo mix and this audio card is god knows how old
speccy just says "High Definition Audio Device" and I don't remember installing any drivers
If you don't have stereo mix, use "Wave Out Mix". It should do the same thing.

I really wish manycam had an additional audio option, then I could just play audio through that + video

sadly no, for that you'll need virtual audio cable
if you own an external soundcard, you can route your audio to that and from that back into the line input of your pc using audio cables.
I use this way to setup my dj gear for online streams and stuff like that.
