What in video game history has caused the biggest dissent in either the gaming community or society in general?
Right now Gamergate is definitely going to be quite a defining moment for not just gaming, but for journalistic media. Plus gaming in general has been getting both more and less controversial as it's become less of a niche medium. The shit that has been flung in Gamergate is what has been seen in most creative mediums when people join for control and money as their main reason for being a part of it, instead of contributing to the space. The overall good games will be made by people who put their "politics" behind for their work.
gamergate doesn't really have much to do with video games, it's just a big stink in journalism that only sjws and gamers care about. it's not really talked about all that much in the mainstream media Hot Coffee was only really a big deal among people who already found the game to be objectionable. Nobody but a few angry nerds cared about the end to mass effect 3. anita sarkeesian isn't a gamer or a video game journalist and is only relevant in the context of gamergate. I've never even heard of aliens colonial marines the biggest stink in video game history was when the industry crashed in 1983. sales dropped 97% in 2 years and we're lucky that video games exist anymore.

When GTA 3 came out, the media were instantaneously drawn to it, Columbine was a recent memory. As much as people like to toot the Feminist/Gamergate/Sarkeesian thing, it's small fry compared to the shit that happened back then, most people don't give a fuck about the nonsense that happens today with that stuff, it doesn't spark the same family values and moral panic that it did. But back then it was the hot topic like Heavy Metal in the mid 80's. The stink we are seeing right now will never get much beyond what we see now, it's a combination of "internet" Jurno's covering their asses, hateful bigots spouting bullshit for cash and attention, and sexist, weirdos getting mad because they are easily lead by idiots. It will never be relevant in the grand scheme of things no matter how bad each party wants it to be.

I was considering putting the 1983 crash but that was less of a controversy and more of just a market collapse

Hell, people expected it to be shit the instant the leaked footage hit the internet and later when all the content footage was uploaded after 3D Realms had collapsed. Colonial Marines came out of nowhere as a shit game. It was in development hell for a little while, sure, but they outright lied in the demo and the promo stuff, DNf was pretty straight forward with how garbage it was.
Mortal Kombat and The ESRB in general
the ESRB and the Video Game Crash of 1983 most of the ones you have in the poll really weren't that big in the long run. They were big for the time, like Gamergate was massive for 2014, but as far as gaming as a whole? They don't really compare
No mention of Doom, GTA III, Night Trap, or Mortal Kombat Those are pretty essential for a video game controversy list.
I think the fact that Alien: Isolation didn't sell more should've been a bigger controversy :^)
Ubisoft. How its more believable to them that everyone just pirates their games rather than admit that maybe they don't appeal to the PC crowd and/or are too similar to the console version that they may already own. How they don't need to quality test their PC ports, people should just buy the most expensive graphics card. How its NEVER their fault. And worst of all, how they somehow fucked up TETRIS and tried to claim that it was messed up as TETRIS is apparently too complex a game for the PS4.

Gamergate is certainly one of the most controversial, but it hasn't really had a massive impact on games like things like the ESRB or 1983 crash did.
The only one that I ever paid any attention to was the 'No Russian' mission when MW2 came out.
