I would say Doom, but I don't think video game ratings existed back then. So maybe it was Mortal Kombat. Isn't that the game that caused video game ratings to come into existence? At any rate, I don't think Mortal Kombat was rated M, because there was a different system for several years which would've had MK classified under some rating like "MA-13." E10+ is kind of retarded, I think. Then there's also the occasional obscure game that's rated E and also says "6+" right below the rating.

I dunno actually, but I think the first PEGI 18+ rated game I played was Mass Effect, though it might have been Assassins Creed. (yeah, I'm just not one of those persons that started playing shooters at age 6, sorry)
