Most companies use some other symbol instead of @ for example. (Though + would also cause problems.) Placing a completely fake symbol somewhere in the handle (or registering the accounts in question) would probably work best though.

I don't see how EA or any media outlet could evade it, using random words to make a twitter handle is a lot more immersive than just writing out many different ways to say John Doe ( like :@JonDoesey or @JovenDoDo ). However that would be repetitive which is why many people use a name generator or pick names out of a hat and use it; Which is why whenever fictitious names are used, there are usually disclaimers reminding everyone of the chances of said names in used in these mediums purely coincidental.

Reports like this rarely occur in the world of media, but coincidences like this may very well happen more than often than we would likely already know. When using random names you cannot readily predict a combination being associated with an actual alias. You could use a google search but all that would do is give you a bunch of names with the same permutations and variations as any other name would. You could blame EA for being lazy and I could see that blame to a certain extent; However it's unfair to say that they specifically targeted this man in particular when a hat pick can very much just be a cannister of coincidences.

Mind, I agree the way this person handled the situation only made it worse for themselves in the end because the internet is full of trolls, but I can see why someone would be rather pissed about this happening.
