Why is it that for years even midrange graphics cards, not just high end/enthusiast cards, only have DVI Ports as their primary output (not counting S-Video/HDMI/AV) but a large majority of LCD screens still only have VGA D-Sub output?
Even the ones that do have DVI usually don't come with a cable for it, unless you buy an expensive one.

I've only seen very low end LCD monitors supporting VGA. It's most likely cheaper.
The DVI ports are backwards compatible and almost every video card will come with a DVI to VGA adapter. As for monitors, every one that I've seen that is bigger than 17" has DVI input and came with a DVI cable.
