I was thinking about which fictional world would be the absolute worst place to live in as a civilian.
Azeroth is a prime candidate. It seems like everyday you would run the risk of being infected by some plague; or having a demon invasion on top of your farm. There's always some evil shit going on.

How the fuck can humanity supposedly survive a nuclear apocalypse and yet hundreds of years later you might have a house or an office but you've never bothered to clean out the rubble strewn about or fucking attempted to rebuild civilization?

I'm not quite sure it's fair to pick universes that are basically dying or in that case are dead beyond a very small handful of people. You can't really live in those.

I mean, I could say a universe like Casshern Sins's one where every living being including robots has super-cancer so literally everybody is dying, and I feel like that's kind of cheating.
I'm having a tough time deciding between Warhammer (40k and Fantasy) and maybe Fallout.

You're thinking of the east coast (Bethesda). On the west coast (Obsidian), whole nations have been founded and built on the rubble of the old world. The quality of life can vary greatly depending on the environment you're in, and everything's a gray area. For example, if you're in Legion territory (figuring you're a healthy, adult male), you could live a fairly safe and decent life. Although that comes with the con of being bred for war from a young age, and possibly dying in combat later on. There's also the watchful eye of the vulpes inculta around every corner, so who knows if you'll be accused of doing something that goes against the Legion.

Either you get killed by xenos, or, you're born in a hive city (think of a massive city confined to a massive mile-high building) at the very bottom of it, and you die because you decided to step outside for once in your short, miserable life, and a passing rape gang chopped you up and skull fucked you to death. Almost literally anything could happen if you lived in the Warhammer universe. Tyranid swarms, massive planet-wide rebellion, someone decided to mess with the warp which invited Chaos to your world, which then subsequently sanctioned an exterminatus on your planet. You could be one of millions or even billions killed an instant over absolutely anything.

Depending on where you live the 40k universe isn't all bad. You could live a decent-good life in some of the hive cities/worlds.

Halo is a pretty fucking terrible universe for nearly every sentient being possible. The Forerunners and Precursors are nearly or entirely extinct. The various Covenant species are either cannon fodder, corrupt, or fixated on honor to the point of suicide. The UNSC is sitting on the very edge of the moral event horizon with a large swath of humanity brutally murdered over twenty-seven years.
It's a shit state of affairs regardless of what species you are.
