I need a decent mouse that has nice features yet doesn't cost fuckloads. I would buy a G5 but it's still fuck expensive where I live, so I need something with similar capabilities but lower price.
The Microsoft SideWinder X5 looks pretty decent, is it worth the money?

Looks pretty generic. I need something more ergonomically designed and with a few extra buttons, the sidewinder seems to be what I need, but I'm not sure if it's really all that it says it is.

Get a Logitech Trackman.
They save your arm all the pain of moving.
You can get used to using a trackwheel very easily.
They're cheap, too.

Get a refurb copperhead. Got mine for 25 about a year and a half ago. Check some tech deal sites theres always some mouse discount.
I don't really like Apple, and really don't like Macs, but that one apple mouse is pretty nice. It does everything you need, looks pretty nice (my friend who owns a mac spraypainted his black because it looks better), and has an extra button. (It's really two buttons that you have to press simultaneously, but you get the point.) And it has a 360 degree scroll wheel, which really comes in handy.

I have a Sidewinder and I love it. I don't use the DPI on the fly or the macro button, but I love this mouse. I will say it's a large mouse, it holds my entire hand where other ones I'm left floating. I would buy this mouse again if I had to, but it's held up.
I have to say that I have the $80 version with the metal scroll wheel and side buttons.
I got the Sidewinder X5 as a replacement for my habu. Really great, takes a couple days to get used too but it's really comfy.
