MKAY! So I just reformatted my PC around 4 hours ago, it's running Windows XP. Everything was going fine, I installed CCC(Radeon HD 3850 AGP)
I was pretty much finished installing my programs, and I didn't play with any settings for my graphics card.
I decided to launch CSS and see how smoothly it would run.
Turns out it was horribad, I keep getting graphical fuck ups:

Ingame it's WAYYY worse.
Also it occurred in Gmod
I'd say your graphics card is fried or damaged, same thing happened to me. Once I installed a new graphics card the problem was fixed, so I'd try that if nothing else works.

Highly doubt that, always 50-71 degrees.
I keep it fairly clean.

check your display properties and make sure your graphics card is checked under "settings"

Did that 4 times, I should also mention i deleted all my steam games except Garry's mod and CSS, I backed those up, do you think thats the problem?
I have an old 8500Gt (passive cooling) and its like always at 65°C - 75°C .. and my graphics arent fukd up like that
maybe u should get a custom cooler for ur gpu
Just because the temps are normal doesn't mean the card isn't failing.
You could always just re-format again. If that doesn't fix it, then your card is dying and it just happened to go during the format.

I suggest get a new card. Like another user said, it's probably fried or damaged.

What does your 4870 have annything to do with his card?
OP:
1:Check your temps again.
2:Use drive sweeper to remove your VGA drivers and reinstal an older version of it (note that CCC is not a video driver)
3:Make sure all your cables are still plugged in and maybe re-seat your gpu.
try using the old steam UI

You fuggered up your 9800GT? Man, what the hell did you do to it? That's one hell of a sturdy card in my experience.
A new problem arose, there are some other epic graphical fuck ups, such as when I'm in Jailbreak electric, the server works fine, but I cant see though transparent objects(Cells, Big cage, Water) ect.
So I tried to go in CCC and this is what I got:

I had a similar problem where textures were wrong or sometimes garbled. There are a couple of things you can do.
1. Look up a tutorial for baking your graphics card in the oven. This is a [b]TEMPORARY[/b] fix.
2. Bite the bullet and buy a new card.

It doesn't appear anymore O_O, I restarted it still happened, but I reinstalled the drivers and everything was happy (Insert Pacman emote)

I just played around with the drivers. A lot.
This thread tl;dr, in case someone hasn't said it yet. Overclocking causes problems for me in source games on older drivers, it may for you on new drivers or old drivers, leave at stock speeds if you have it OCd
