I think it relies on the tf_forced_birthday command instead, since afaik tf_birthday doesn't trigger the holiday event, just the hats/gibs/balloons and things.

I think It stays forever... Why someone would want vanish your items from your backpack (unless you are a hacker)?
the items stay forever, but I'm pretty sure the noisemaker doesn't work anymore
