Nothing, but if you hit an enemy with it a killicon styled notification appears in the top right corner, and it shows XNUMBEROFHITS for ever hit you deal to a single person

The game I was in had lots of people with the new stuff. The guy with the Mackerel said it's not different except for the fact that it announces how many kills you've gotten with it (x6, etc.) with each kill. It also says FISH KILL! on the kill icon.

Black Box+Backup is awesome, you can take a bunch of damage, fill your rage meter, use the Backup, than get all the health back with the Black Box.
