As of last year you can go as high as 512X512, check out [url=http://www.the303.org/tutorials/spraylogo.html]my tutorial[/url] for more info. If you specifically need a full chart for animated sizes, check out [url=http://www.the303.org/tutorials/tutorial03.html]part 3 (animated section)[/url]

Not exactly, usually the checker pattern happens when the VMT has the wrong path written in it or a name mismatch. A second problem is that you made the VTF as a 7.5 version which is incompatible with the TF2 version of source (check your advanced VTF settings in vtfedit to see this when importing). Path should always read as:
"$basetexture" "vgui/logos/nameofspraylogo"
Another common mistake is placing everything in the "vgui/logos/UI" folder, or copying the vmt to your folder and not the vtf.

I haven't used any of those Highfps configs, but "No level of detail" flag on your vtf should be set (this overrides the game's lower quality texture settings).

Not sure what you mean by "decal number". Again, if its blurry because your game texture quality settings then "No Level of Detail" VTF flag is all you need.
