On the Hale server I play, every shot to Hale with the Bargain counts as a headshot, so it's quite useful there.
But regular gameplay? Oh my yes, it's appalling.

The fact that a miss only loses 1 and a bodyshot loses 0 makes it less terrible, but yeah, it's not that great.
Also the glitch that allows Eyelander/Headtaker/Nine Iron Demomen to steal the heads stored in the Bargain is just sloppy on Valve's part.
