And use pyro airblast to counter ubers, backburners are good for catching the medic before he knows what hit him

Hah, i just took down a heavy/medic combo that had been hindering my team for like a minute with a quick degreaser/axe kills.
Protip: Go for the medic, i saw he was charged, if i had attacked the heavy first he would have released the charge.
Use the Huntsman taunt on an ubercharged Heavy or Medic. Observe the rage.
But srsly, sniping and backstabbing are the easiest counters for a Heavy/Medic pair, especially Snipers since they can keep their distance.

You guys are acting like these heavies won't be using the natascha; I haven't read a single "strategy" yet that can deal with natash effectively (except for plain' old sniping, but even then you can't do anything else BUT snipe)
If you're a good Spy, the Heavy won't have time to react when you stab the Medic.

If the medic isn't a retarded kid he just has to watch his back and not get stabbed, if the heavy isn't a retarded kid he'll hear or see your kill and rip you to ribbons - he just needs to be ON par to realize you're using the eternal reward, too. Honestly, it's like some of you have just bought the game.

Fight fire with fire, meaning you could go Heavy/Medic too. If they're using Natascha, use your default, and you'll rip through them before they can to you, if not, you'll have to surprise them somehow. If there is more than one Heavy Medic combo, either get that Uber flowing before they can, or get some other teammates to help out.
Surprisingly, Team Fortress 2 is about teamwork.
There's been a trend of most "pubstars", the guys who are top of the scoreboard by 20 points with 8 dominations have been playing exclusively Heavy with a pocket Medic. I very rarely ever see pubstar Soldiers which used to be the class of choice, rarely see Scouts or Demomen either.
Which is sad because all of those other classes were fairly easily counterable. Heavies with Natascha are pretty much only counterable by other Heavies with Sasha at this point I feel and to an extent Engineers.
Another thing that really really pisses me off about Heavies is the crits, if a Heavy crits and he's shooting anywhere near my general direction I'm dead within a second. They also do pretty ridiculous damage normally.

Best thing that works as spy is trying to find a place to jump down from above, heavies are usually linear, if they are surrounding a cart or something, there are usually vantage points for spies, take your eternal reward, jump down ontop of a group of heavies/medics that are shooting in different directions, take out anyone you can before you get noticed. This worked for me on goldrush, jumping down from the side building at 3 heavies and 2 medics, took out both medics and 2 heavies.. Even that is good enough for your team to take out the last remaining few.
If you want a more linear approach, if there are heavies firing in all directions, chances are they are going deaf, and are not concentrating hard enough, so with use of dead ringer, run up to the heavies/medics, obviously get shot at, drop a body, run up invisible and try to squeeze in between them , decloak in the midst of them, they could hear you, but it's harder to notice the sound if you have a fez/when they're all spamming their Natashas and Sashas. You could take out a few before dying.
Obviously works best when they are distracted.. As how spies work in general.

Competent heavies notice the 'Your Healer:' sign disappear and immediately turn around. And you're also assuming that there is only one pair where the OP asked for counters against groups of pairs. The OP also said that the heavies are apparently facing and shooting all directions forming a ball of death. The eternal reward won't protect you from that.
From my experience the only true counter to groups of heavy medic pairs is team work. A competent sniper may do fairly well, but after the firing cone tightening the heavy got they can screw up your aim half of the time.

Your healer sign will disappear and reappear constantly as medics switch healing to different heavies and other medics, they won't freak out every time it disappears. I feel its better to go after the heavies first as a spy, your teammates can take out medics.

True but if it's just the pair, not a group of team mates where you will generally be most safe, they tend to spin around every so often.
Isn't the OP about when there is a group of enemies?

It is a good idea to go for the healing target only when it's a sure one-hit kill. Heavy goes on fire? Ubercharge deployed, pyro shredded to bits.

as long as the heavy isnt ubered, 1 crit grenade/rocket can make all the difference.

Kritz soldier can be a great counter to a group of heavies. 2 rockets right in the middle of them and they're all dead.

Yeah, I'm rolling off topic. But if the group is watching vigilantly, and keeping tabs on their medics, the spy may have a hard time.

As a spy I rather play support and save my teammates, If I take out the medic, the heavy can still damage my teammates, if I take out the heavy, my teammates can kill the medic. But Normally if I see the opportunity I go for Medic then Heavy backstabs. Usually only with pairs.

Well stopped healing and that sign, how long would it take for a heavy to realize his medic was backstabbed when he's firing at enemies, if they're in a group, stopped healing sign would be constantly going on and off so they wouldn't think much of it, however if they are distracted how long would it take for them to notice the backstabbed your medic sign? And does the backstab sign even show up for enemies if you're using the Eternal Reward?

Well yeah, but groups are rarely in tight communication with each other, the only reason groups happen is cause they feel they are indestructible if they roll on 5 heavies and a couple of medics onto the cart. With rockets and such things flying at them, everyone shooting in all directions, it is very hectic for them to keep track of which medic is currently healing them and which is not.
Just wait for distractions if you can't chainstab on your own.
