Probably late but, I can fully say that the m_? thing is "M_M" because if you look, there's two white lines in the same fashion as the "M_" I had to zoom in to be sure about it.

The group that profile is in ([url]http://steamcommunity.com/groups/nohumansallowed[/url]) was made on June 22, 2011. Exactly 1 year before our current best guess. lots of coincidence.

I can confirm that the new items usually drop with something else, so I think they just attach themselves to drops you get... making them unbelievably common.
