Because A) The people DMing probably don't care that MONOCULUS! would get stronger if he escapes, and would just keep killing anyway, B) The people unable to kill MONOCULUS! the first time would be pretty much screwed, and C) MONOCULUS! leveling was added after the initial release for the people who thought MONOCULUS! was too easy and wanted a bigger challenge.
On the topic of whether MvM was supposed to be RED vs. BLU, the RED Engineer was pictured building what we assume to be the robots. So the plot must've been pretty thick already at that point. Involves the RED Engie being a double agent for either Gray or BLU.
I 'd love them to add the nifty "See friendly ammo count" to the Medic's core mechanics that is present in MVM but no in normal play.
So it would become possible add a downside to the Amputator and Solemn Vow being "Cannot see friendly ammo count while healing".
And a new bow for Sniper.
And a fix to the Mini-Sentry not having a smaller range, respecting the 3D Sphere (detection area) being smaller then the stock. And a new Engie PDA!
And a new Demo set with explosives!
And... Let me think about it. I'll come back to you guys.

they promised a fix for Mann Up rewards when the item server is down this week, they need to deliver because this is actually probably costing them money

The Sticky Jumper is all I'll ever need. I could never get the hang of using two primary weapons with Demoman, so being able to have super mobility as a trade off to not having to worry about two weapons is why I like it.

That has nothing to do with it actually being considered fixed. He said that it's being looked into.
Unless you have some sort of confirmation from him or another employee stating otherwise, it's not considered to be "fixed" just yet.

Those blueprints were from Radigan Conagher, Engineer jus pulled them out with the other blueprints. I'd say it much more likely he helped Gray build the bots.
