Id make a Doom one of course.
Alot of reskins, like the Rocket Launcher, Pistol, Shotgun, etc, but Id also add a Hand mounted rocket launcher like the Cyberdemon's.
As for cosmetics, id have an All class Doomguy helmet, A Berserk pack misc to sit on places like the Engineer and Medic's belts, and I wouldnt mind a smaller toxic barrel to be strapped to characters back or whereever theres space that has the same effect as the Bombnomicon when you die, except green.

As much as I hate companion items, a little Cacodemon or Lost Soul following you would be neat.
