I use a custom HUD too, I forget which though. Not at my home PC so I can't check or anything. Anyway, I prefer to use it because it makes things larger, yet they cover a bit less on the screen. I'm not a comp 2pro or anything, I'm just a spaz and if I need to move my eyes too far away from the center of the screen while I'm fucking around as the Scout or Engineer I might just puke.
I use a custom HUD but only because I like having the STEAM avatar beside the name of the person on the mic and also to see the actual hp numbers of someones health. I wish I knew how to edit the default HUD to add these two things.

I use the PVHUD, which as that neat self-updater feature (dunno if others do) and it's constantly updated when TF2 does. But yeah. It's all about individual tastes. Mods? Some. Like the Death Ray for the CM5K which opens a lot of the view, a few model fixes by the Emporium (amazing) people, a few sound mods and of course, particle mods from Chaofanatic.
