Put it into team fortress 2/tf/materials/console, it must be a .vtf to work.
I think existing backgrounds you can replace with your own backgrounds are background_2fort, background_gravelpit, background_upward, background_mvm, background_xmas2011, title_fullmoon, title_team_halloween2011, and title_team_halloween. Keep in mind every one of these has a widescreen variant which is just a horizontally compressed version of the original.

i feel it doesnt fit the rest of the outfit. Sorry. I think its just me though since i know that samurai don't wear their hakama to battle with their kabuto's. Extending the demo's pants with all that team color downward kinda breaks the colors scheme too.

but now that we know we can flex we can cover up the cigarette entirely, only problem is you have to match up the smoke, which shouldn't really be a problem since your pipe doesn't curve

Could the belt perhaps be some other color? I'm not saying it's horrible, but I do think that maybe there would be some color that matches Spy's colors more naturally.
The brown color just sort of sticks out a bit. The buckle is excusable since it's such a small part.

The way his body bends makes it difficult. We had to remove the buttons from our spy item cause they refused to work.

Its not hard, its impossible get those not going through the spy mesh in many animations, better get inside the spy than stick out/float imo.
