I only had Empire at first and didn't get anything. Then, a friend bought me Napoleon and the Medic coat and shako dropped for me. As I said before, no signs of the Spy coat so far.

The spy coat is - rather stupidly - attached to Medieval II as opposed to Empire or Napoleon.
