You open up a map and put Sv_cheats 1, Thirdperson and Cl_drawhud 0 in your console. Then you mess around with cam_idealyaw (Rotates the camera), cam_idealdist (Changes the camera distance), cam_idealdistup (Moves the camera upwards (downwards if you put in a negative number)) and cam_idealdistright (moves the camera to the right (Left if you put in a negative number))
I often put in Cam_Idealyaw 150, Cam_idealdist 40-50, Cam_idealdistright 0-20 and cam_idealdistup -10.
And voila, it looks slightly similar to this.
