Probably because the cap does a good job at hiding the edges of the mask and therefor it looks more like his face rather than a mask, and the tiny horns sticking out doesn't look like obvious clipping. The misc item looks good with it because it fits with the color of the cap.

Fun fact, I equipped that loadout instead of my usual (Loadout A, Scatter, Luger, Atomiser) because I had a Scattergun skin. Why do you guys pay more attention to the weapons I equip anyway.
