It's a 7% margin, so pretty much any slip up lets them catch up.
Also Flamethrower range is weird.
Should I feel bad if I'm using a "crutch" loadout, e.g. the Spycicle, the Enforcer and the DR for Spy?
The very reason they're called "crutch" is actually because they inhibit your ability to improve skill. They help the unskilled perform better, but you won't get better using them, and to the already skilled they just slow them down. So for example, Dr. Enforcicle allows you to escape damage unharmed, meaning you don't need to be very cautious, and so will never learn to be a careful spy like the best spies are. It also puts emphasis on your gun rather than knife, which is supposed to be the main weapon you learn to use.
So basically, don't feel bad, but you can be better at the game if you ditch it. I'm glad I ditched the LL.

Throw out the DR and Enforcer, go for Invis and Ambassador. Just my personal opinion.

If I stand near a dispenser or the bomb cart, will my overheal decay? I haven't played the game for about a few months and I've only recently got back into it. I'm a little out of the loop.

Why are you asking us? Mods are all about personalising your game and whatnot. It's 100% up to you really..... Unless sv_pure is giving you a hard time.
Back on topic... Any tips for playing engy on attack without a gunslinger? I just feel too damn cheap using the thing. And when I use my regular sentry 100 uber demos and soldiers tear me limb-from-limb; even if they didn't know it was there before or it's only level 1.

That's exactly what I do. I actually like using the jag more than the gunslinger, because it allows me options to either turtle or pop a turret up in a random spot.
When on offense, teleporters are more important than sentries. The only reason your sentry gun is there is to protect your teleporter exit.
When on defense, you have the choice between roaming and turtling. If roaming, set up your sentry in a different place every time and stay on the move. If your sentry stays alive for more than 30 seconds, start upgrading it with fallen weapons. Keep your dispenser near the front lines out of enemy view. If you opt to turtle, keep all 3 buildings within close proximity and place your nest where your team can easily support you.

This is surprisingly effective.
I've been able to use it on badwater defense to get my level 3 all the way behind BLU frontline, almost completely elminate it and wrangle to destroy a teleporter nest from beyond their sentry's range. Then I moved it up to the basic 1st cap defense position and spawncamped for a bit, until my team backed me up. We secured the area and kept it that way for the duration of the round.
Shouldn't be too hard to do on a basic public server and with a bit of luck and balanced teams.

Like Hein said, turn off mouse acceleration, and get a gaming mouse that has a custom DPI. Like on mine, it has 3 modes, and I have each one set for different things. One is for FPS games, one for every day, and one I have for everything else. Just don't get frustrated when you first get it and set it up, it takes time to get the proper DPI that is perfect for you. Everyones different. I have mouse accel off, raw input, cranked up to max. You can see footage of me playing in the What You Love thread, and I have it set to where I barely touch my mouse for it to move. When you play heavy as much as I do, you sometimes need that quick snap speed to get that annoying bastard scout out of your non-hair. But like I said, that's me. You might like something less sensitive.

Ping =/= how well you are connected to the server.
Even with a low ping, you could be experiencing lots of choke and packet loss, which leads to that awful rubberbanding effect.

I have this problem with hitscan, ever since I started maining soldier I can't play hitscan classes. I always instinctively twitch and shoot where I'd normally want a rocket to go. And miss by 3 meters since hitscan has no travel time.
So that may be your problem, if not, try changing your sensitivity. Lower is usually better for hitscan (even for scout).
For sniper, try changing zoom_sensitivity_ratio, if it helps.
