How do you make custom ones? Do you save them as entirely new strings in tf_english.txt?

Type status in the console to get your PID.

He didn't. Almost a year ago people knew about the achievement_earned command but no one knew how to use it. I'm guessing Azu read the "how to hook Tf2" thread on SA because one of the guys in their found out how to use achievement_earned. He only wrote a php script to generate the seed (like many others before him did)
When I found out about item_found it was obvious it was the same deal so I tried it and it worked. Although I'm betting many other people caught on about it before me. So the answer to your question: No one found it out first, it was just general knowledge.
The crowbar is just tf_weapon_crowbar, it's in the tf_english.txt
Along with tf_weapon_tranq, tf_title, tf_ball tf_weapon_nailgun and some other shit that freaks people out when used.

They have to type the command in for the message to show. It won't work unless they do.

I've used Wolves myself, freaked everyone out until one guy who knew about item_found came back from idling (it was his clan's Arena server).

Used wrong item code
None of the item codes have spaces, and AFAIK, the OSIPR isn't in the tf_english.txt, you can only use stuff from there.
If you had for example tf_title instead, it would have worked better, 'cause then it would say Has found: Team Fortress
Also, some servers has started banning for using the item_found command.
I used it a couple of times, got a warning, and the next guy who tried the item_found got banned. I managed to make something banworthy without getting banned myself :v:

It's not the same number at the end for every server. You have to input the current map name and personal PID at the time you generate the code.
