There's a normal map applied to the belt to make the holes into actual holes rather than black dot. Also some tiny, tiny texture patterning on the rest of it.

Some small thing: On both, the stripes seem just a bit too stark. A bit less contrast would be good, all stripes and plaid stuff in tf2 is really low key.

make 2 textures, one blue and one red
if you're making it fully paintable, set the preffered base RGB value (red/blue) in the importer and have a grayscale texture file

add particles like you would a model, click the + and "add particle effect"
it should be under cig_fx or something, then choose the smoke particle and set it to 99 durations (so it's always on)
then lock it to the cockswain, position it if you need to and then scrub the timeline to black (-1.00 in timeline) and back (so the particle "loads" into the scene)
you lock the particle just like a hat to a head, lock the transform/controlpoint0 thing to the cockswain and zero it in

I hope you're aware of the difference between a having a buckle as a subtle detail and having an entire belt on around it. the whole point of the hat is to be something between a bonet and and regular cap.
desing wise, the belt itself its quite an eyesore and makes no sense at all unless it was actually glued to the fabric of the hat. it actually makes the hat look rather cartoonish. mixing both leather and fabric also makes it quite hideous and way larger than it should be(wich is the reason in the original one the buckle is just a detail on the strap of the hat).
I'm pretty sure that at this point feedback implies a scrapping your concept altogether and you probably do not want to work on the subject anymore, but consider taking another shot at the item somewhere in the future when you can look at it with with a more critical point of view.

that's just on the wrap, i left it so it has a paper wrap, but i might fix it to be full instead
