Here's what I think:
if someone thrusted something as blunt and (not really sharp) as the end of a bonesaw into your back so that it could come off the other end, then they would have to use a lot of force... something which is not reflected in spy's pose. His back should be arched way forwards.
There should be more tension in his pose overall (shoulders for example).
Then I guess it's a matter of opinion but considering the goriness and brutality of this I'd use more dramatic lighting. Number one rule of lighting either way: it should never come from the same angle as the camera, or it flattens out everything). Lower the tonemapscale (i suppose this is SFM) and make the lights come from either side.

I was going to suggest that, too. May I add that medic's right shoulder should be thrusted as well, and therefore his other shoulder will have to pull the spy to reflect that.

Unless the pose was supposed to be after the medic finished stabbing, and was about to pull it out. In which case, make the grab shoulder push a bit
to be honest there is no way the ubersaw could go thru his torso without applying some brutal force, a force so powerful that the spy's torso(and expresion) would be completely torn apart as soon as he went past the needle. something like the ubersaw is designed to cut and puncture a body but hardly to pierce with it(it would be the same as trying to pierce a body with your fist).
you could replace it with one of the sniper's machetes and get a better result.

It was posted an hour ago to the SFM posters thread. Apparently it's "loadout for two friends", so I don't think he copied it. Besides, 'Medic stabs guy with ubersaw' isn't really a groundbreaking new concept worthy of copying.
Something I find that a lot of these nicely done SFM posters forget is the [url=http://en.wikipedia.org/wiki/Rule_of_thirds]rule of thirds[/url]. It's one of the most basic elements of photography/visual media and following it improves something tenfold.

Large overestimate. Rule of Thirds is a good template but it does nothing if your picture is just a character or two, because you'll have to compensate empty space
It improves it, but the eye can still easily focus in the middle fine

I'd say focusing on the middle works for some compositions, but the majority should follow it in some way as it does make your image look a ton better. For example, I've overlaid a grid over Elder's image:

Both characters are slap-bang in the middle. This can feel unnatural to look at as your eyes automatically fall to the right or left of the image. Now if we move them over to the right a bit... (Sorry had to enlarge it a bit)

It begins to be a lot more easy on the eyes.

And all we have done is put the right line over the medic. It doesn't matter that spy is in the center as Medic is the main focus here.
Although you don't always have to follow the rule of thirds, it's usually a lot nicer to look at an image if the subject is placed off the left or right a bit.
