I'm suddenly curious, what exactly does it take to have a developer get a promotional item in TF2 anyway? I mean, they obviously have to talk to Valve about it, but is there like some kind of requirement? Like, is there some huge fee to get it in, does the game have to be big a big, expensive one? Can Indie Developers apply to have promos made for their game?

Binding of Isaac: Rebirth got a promo, so I don't think it's too expensive to get an item in the game, if there's a fee at all.

Gunpoint has a promotional item and it's a rather small game. So no, there's not a size requirement to have a promotional crossover.
What [i]is[/i] left in the dark, however, is scale. Realistically a small-time indie team won't be able to create much more than a cosmetics or two, while a large team (say, Eidos or Squeenix) has reasonable manpower and capital to create [i]stuff[/i]. That's not to say that there's a fee to get those sorts of updates off the ground, but that the team needs to "wow" the TF team into agreeing to the proposal and then doing as much of the legwork as they reasonably can before handing the finished content over to Valve to do with what they see fit.

I know that sometimes they hire members of the community to make some of the content, like the Triad Pack.

That's no surprise at this point. Other game developers tend to have their own styles that they're used to working with, it's far more common for an outside company to make something that looks horrid in TF2 because it's got drastically different colors and lighting. It's far safer to go with community members because they're versed in that, they understand TF2's aesthetic well enough to fit the palette and theme. Plus it doesn't cost time and resources in order to train someone to fit the style, a community member can get right down to it.
There's another thing as well that players don't tend to acknowledge. It's a cross-[i]promotion[/i]. Speaking strictly as a businessperson, there needs to be some incentive to agree with the proposal. Valve gets 2 forms of revenue through promotional material: either direct purchases from the Mann Co. Store or Steam Purchases of the game in question. In cases where the content goes beyond the norm of a batch of cosmetics, there typically needs to be some sort of payoff for doing that extra legwork.
Actually, I do have one major complaint about the Gun Mettle update and that is the significant FPS drop I felt playing in any of the new maps. I'm sure it wasn't just the new maps rather the mechanics of the game but I never really bothered to find out exactly why I couldn't play this massive old turd any more.
I was so excited for the eventual Spy vs Engineer update, too.

Who knows, maybe they will release it in the future as another "campaign" with spy-engi specific contracts. Or maybe even like Demo vs Soldier update with a counter of the kills and all that cool stuff, that'd be great but not very likely is it. :(

I am bit late to the cross promo talk but I will put my 2 cents. I know small indie studio in my town, and they were looking at creating cross promo with tf2, the only main offputing reason for that was how awkward is to work with source engine, nobody in group could import anything into TF2 engine without issues and they had no time to spend eternity on learning source engine.
I am not saying they are experienced or masters of industry, they are in majority students of game dev or self taught and nobody is bothered anymore about source engine while you can have Unreal 4, Cry Engine or Unity.

Neither any of them or I by myself managed to import anything into tf2 and making it work. Somethign was always terribly broken.
