The medals replace the earbuds, not sure about the hat, it currently replaces the bill's hat.

This was an attempt to make the Natascha icon not so tiny by hiding useless parts of the icon, like with the flamethrower icon. It kinda turned out shitty though.
It looks a little too detailed, but other than that, it's awesome.
