Call me an idiot, but what does a middleman actually do?
Holds the items until the payment is processed and confirmed. If the payment doesn't go through, neither do the items. As soon as the payment goes through, so do the items. They must have a good reputation and may or may not take a small cut. And items only middleman simply takes all of both side's items and dispenses them. If either side doesn't give all of the items and disappears, then they will be out of the items already given without any recompense.

Oh lol if the middleman was my friend i would tell him to keep the items.
That's why they have to have a good reputation. Many middlemen actually get more in gifts and in cuts in a week than they would get from deciding to keep the items off of a trade.

Also, i'll explain how it works on example:
-player A trades his Unusual Hat to the middleman.
-player B trades his 30 keys to the middleman.
-the middleman trades the items to their new owners.
And both sides are happy.
I found a guy yesterday with about 30 hours played, around 20 items in his inventory and 2 (!!) Hard Counters; no other hats at all.
The chance of finding two of the same hat in 30 hours must be miniscule!
Anyways, he didn't have them equipped and said he'd take anything for them, so got them both for a flare gun and a scrap metal.
Now I have 3 Hard Counters D:

Haha I'm only kidding, although generally these people only have one or two hats i.e. poor.
It made me think of it yesterday when I gave someone a medic mirror for free - it was their favourite class and they only had like 2 hats.
